Medical and Dental Professionals

The Division of Oral Medicine and Dentistry at Brigham and Women’s Hospital is a leading comprehensive oral medicine clinical service drawing referrals from throughout New England and beyond for diagnosis and management of oral medicine conditions. We are dedicated to working cooperatively with referring dentists or physicians to provide oral medicine and dentistry services for medically complex patients. We provide timely appointments and can generally accommodate an urgent or emergency referral. Please call (617) 732-6974.

How to Refer a Patient

Referral Process

• Fill out the Oral Medicine Consultation/Referral Form
• Ask the patient to fill out the New Patient Medical Intake Form and bring it to the first appointment.
• Ask the patient to bring to the appointment:
• The Oral Medicine Consultation/Referral Form
• All pertinent tests/results:
• Biopsy reports
• A summary of medical problems, medications, and any other relevant details
• Radiographic pictures and reports
• This information can also be:
• Faxed to (617) 264-6312
• Scanned and sent via email to bwhoralmedicine@partners.org
• Have your patient contact our office to request an appointment.
