This means that all members of the staff at your primary care practice work together to manage and coordinate your health care, both during and between your visits with us.
Care Team members may include your Primary Care Provider, Physician Assistant, Nurse Practitioner, Medical Assistant, Nurse, Practice Assistant, Social Worker, Health Coordinator, Nutritionist, or Pharmacist; the Care Team works together to address your unique health care needs.
