John Fanikos, R.Ph., MBA, is Executive Director of Pharmacy Services at Brigham and Women's Hospital in Boston, Massachusetts.

Jean M. Connors, MD is a hematology attending at Brigham and Women’s Hospital and Dana Farber Cancer Institute, the Medical Director of the inpatient Hemostatic Antithrombotic Stewardship program and the outpatient Anticoagulation Management Services, and an associate professor of medicine at Harvard Medical School.

Samuel Z. Goldhaber, MD, Professor of Medicine at Harvard Medical School, is Interim Chief of the Division of Cardiovascular Medicine, Section Head of Vascular Medicine, and Director of the Thrombosis Research Group at Brigham and Women’s Hospital.  He is Medical Co-Director of the BWH Anticoagulation Management Service.

Katelyn W. Sylvester, PharmD, CACP, BCPS, is a Pharmacy Manager at Brigham and Women’s Hospital in Boston, Massachusetts. Katelyn is responsible for managing the outpatient anticoagulation management service, inpatient hemostatic and antithrombotic stewardship program, and coordinates the credentialing of ambulatory advance practice pharmacists.
