The Patient-Centered Medical Home (PCMH) is a care delivery model to improve health care. Medical homes provide coordinated, patient-centered care to the patients they serve. The model strives to provide comprehensive, accessible, and quality care. The goal is to help patients receive the necessary care when they need it, where they need it, in a way that the patient understands. The National Committee for Quality Assurance (NCQA) is the most well know PCMH recognition program and there are more than 12,000 practices that are recognized.
