The model strives to provide comprehensive, accessible, and quality care. The goal is to help patients receive the necessary care when they need it, where they need it, in a way that the patient understands. The National Committee for Quality Assurance (NCQA) is the most well know PCMH recognition program and there are more than 12,000 practices that are recognized.
