When you become a patient you will choose a primary provider and become a patient of a physician-led team. Your team will be responsible for coordinating and tracking your care over time.

In addition to the physician leader, your team will include a physician assistant, medical assistant, nurse, and social worker. Also, you will have access to a dietician, pharmacist, and other health professionals. We consider you, our patient, to be a critical member of the team and therefore encourage you to take an active role in your care by asking questions and expressing your needs and concerns.

Also, we believe wellness and preventive care are as important as sick care. Your team will communicate with you before, during, and after practice visits to check on your wellbeing, assess your progress, and help you develop healthy habits. This includes reaching out to you following a trip to the emergency room or a specialist.

Our practice has been designed to be a patient-centered medical home, a new model of primary care devised to better meet your health needs.
