Brigham and Women's Primary Care Associates at Norwood uses one-on-one doctor-patient visits, as well as team-based consultations to provide comprehensive and coordinated heath care for patients and their families. Each patient has a health care team that includes their primary care physician plus a physician assistant, social worker, medical assistant, and licensed practice nurse.
We emphasize overall wellness and preventive care, not just sick care. We develop individual care plans for each patient, carefully tailored to their needs and preferences. The care team communicates with patients before, during, and after practice visits to check on their wellbeing, assess progress, and help the patient develop healthy habits.
