Brigham and Women’s Faulkner Community Physicians at Hyde Park has been partnering with the community for over 20 years. We are dedicated to providing preventative and urgent care services (for established patients), disease prevention, and health care planning by appointment only. On-site laboratory services are available (by appointment) for our patients, along with a patient portal (Patient Gateway) which can be used to request appointments, request referrals, view lab results, and interact with staff regarding non-urgent matters. We also have appointment availability with our social worker.

We are located on Hyde Park Avenue between Cleary Square and Shaw’s Supermarket (across from McDonald’s). Parking is available on either side of the building.
