Adult Medical Department

The Adult Medical Department provides primary care for patients 18 and up.* By providing patient-centered care, the care team works to develop lifelong partnerships between our providers, staff and our patients. Our care teams consist of primary care providers, RNs, LPNs, Medical Assistants, and Practice Assistants to provide comprehensive care. Population Health Managers, Community resource specialists, a pulmonologist, a cardiologist, and a nephrologist collaborate with our care teams to support our patients. Patients receive care during face to face visits with providers, RNs, and LPNs; Home care advice and care coordination are provided through telephone triage and support; and on the internet portal called Patient Gateway. We take pride in providing high quality medical care for the residents of Boston in a friendly, patient-centered environment.

  • Chronic Disease Management
  • Sick visits-Same day appointments available
  • On-site laboratory
  • On-site access to our nutrition, behavioral health, OB/GYN, and dental departments
  • Patient portal (Patient Gateway)
  • Referrals for preventative screenings (mammograms, colonoscopies)
  • Referrals for specialty visits
  • On site Specialty services: Pulmonology, Cardiology, Gynecology and Midwifery

Brookside is dedicated to making sure patients receive quality healthcare and have a living situation to support a healthy lifestyle.
