Dr. Frain joined the division in January 2014 to lead ongoing collaborations with primary care to develop and implement new models of co-managing outpatient geriatric patients. She is strongly committed to improving the quality of primary care to older patients and developing models of care.

Tammy Hshieh, MD

Tammy Hshieh, MD

Dr. Hshieh joined the division in July 2013. She is board-certified in geriatrics and internal medicine. Clinically, she is committed to providing optimal care for the aging population, as an advocate for her patients and preserving holistic quality of their lives.

Houman Javedan, MD

Houman Javedan, MD

Aging/Geriatrics

Dr. Javedan is the director of inpatient geriatric medicine at BWH and was recently promoted to the Clinical Director for the Division of Aging in the Department of Medicine. He is a Harvard Macy Scholar and the assistant program director of the HMS Multi-Campus Fellowship in Geriatric Medicine.

Shoshana Streiter, MD

Dr. Shoshana Streiter joined the division in 2019.  She is board certified in geriatrics and internal medicine. She leads the HOPE (Hospitalized Older Person Experience) pathway, a co-management service between Geriatrics and Internal Medicine, which she initiated in 2019.

Samir Tulebaev, MD

Dr. Tulebaev joined the division in September 2013. He has developed a geriatric co-management service with the Trauma, Burn, and Surgical Critical Care Service and has been seeing patients since October 2013.

KRISTIN CARDIN, NURSE PRACTITIONER

As a nurse practitioner who specializes in geriatrics, Kristin Cardin takes a comprehensive approach to care, which starts with listening carefully to her patient. After graduating from the University of New Hampshire, Kristin began her career as an occupational therapist concentrating in geriatrics. She served as an occupational therapist in various home health and skilled nursing facilities as well as acute care and rehabilitation hospitals in the Boston area. Kristin received her bachelors of science degree in nursing from the University of Massachusetts, from which she also holds a master’s degree in advanced practice nursing.

Prior to joining the Division of Aging in 2010, Kristin was on staff at Faulkner Hospital, where she provided post-operative nursing care for seven years. She received awards for excellence in clinical practice and teaching and for excellence in breast cancer support and awareness. Her vast experience in physical rehabilitation and nursing has made her a valuable resource for information and management approaches relating to the care of older adults.

Kristin provides inpatient consult care and offers home visits for frail elderly patients. She has composed her own mission statement: “For me, the health care field has always been much more than a profession. In fact, it is my passion. My career path from occupational therapist to nursing has taught me the importance of treating the whole person, with a strong focus on improving the quality of life of my patients and their families.”
