The Center for Older Adult Health is geared to meet the needs of patients 65 and older who require consultative care. The clinical team works with patients to optimize overall health and to achieve the goals important to that individual – from helping patients maintain good health to treating acute and chronic conditions. This includes determining the best way to manage multiple conditions, which sometimes affect older patients.

- Referrals as needed.

Establishing care with our providers:
