The clinic provides medical management of patients suffering from Congestive Heart Failure.
The Congestive Heart Failure (CHF) Clinic provides assistance and education to any patient and their family to maximize the outpatient management of their heart disease. The multi-disciplinary team includes a medical director, nurse practitioners, and nurses. The goal is to support our patients, to promote wellness, and decrease costly inpatient hospital stays. The program coordinates patient and family heart failure education.
