The Occupational Health Service (OHS) provides pre-placement health screening for all new employees as well as treatment for all work related injuries and work related illnesses. As part of the pre-placement process, BWH requires a health screening through OHS before your first day at work. All staff are required to be immune to measles, mumps, and rubella. You should bring written documentation of your vaccinations or blood test/antibody titers. The form below may be completed by your health care provider and brought to your OHS visit. If you are unable to provide written documentation at your pre-placement exam, you will be required to be vaccinated at no cost during your exam.
