Inpatient Experience
Typically, Endocrinology Fellows spend 12 months performing inpatient consultations at the BWH (10-11 months) or the Beth Israel Deaconess Hospital (1-2 months) during their clinical training. They perform the initial evaluation and provide diagnostic and management recommendations to the patient care teams in consultation with the attending Endocrinologist. On average, the fellows follow 8-10 patients at any one time. On-call responsibilities are shared among 4 fellows and are minimal after the first year of training.
Faculty members at the BWH are assigned to attend on the inpatient consult service on a biweekly or monthly basis. Teaching rounds occur Monday through Friday and ad hoc on weekends. These rounds are designed for presentation of new patients, discussion of ongoing cases, treatment plans, and for education.
The inpatient rotation at the BIDMC focuses on the management of the pregnant diabetic inpatient especially during labor and delivery.
Outpatient Rotations
The bulk of the outpatient training program occurs at the BWH but can incorporate electives at the Joslin Diabetes Center, Children’s Hospital and the VA Boston Healthcare System. In the outpatient clinics, fellows are responsible for the initial evaluation and diagnostic plan, and follow-up care under the supervision of an Endocrinologist. The primary attending bears the ultimate responsibility for the patient's care. During the intensive clinical (first) year of the fellowship, rotations in the subspecialty clinics in Endocrinology last four months and are designed to provide intensive training in the various Endocrine subspecialties. Exceptions to this are the Fellow’s Diabetes Clinic (FDC) and the Longitudinal Clinic. These two clinics are designed to provide a longitudinal endocrine disease management experience and are mentored by an attending throughout the training experience.
