Care Coordination

As you prepare for your hospital stay, you will find that the professional staff at Dana-Farber/Brigham and Women's Cancer Center have taken into account all facets of your care. A very important aspect of your recovery is a smooth, safe transition to home. If you feel that you will be faced with difficulties on your return to home, please discuss this with your physician and he or she will place you in contact with the Care Coordination nurse assigned to the Gynecologic Oncology service. This same nurse will be available to assist you with an individualized discharge plan during your stay at the hospital.
