Home Care

Once you have been discharged to the comforts of home you may require extra care as you recover there. Mass General Brigham Home Care can provide the assistance you need—from general care to specialized services for managing chronic conditions. Our team of providers, in towns across Massachusetts, provides care part-time or around-the-clock.

Prior to leaving the hospital, you will receive discharge instructions and contact information for your BWH medical team in case you have questions or concerns. Your transition from hospital to home will be arranged by a care coordinator—a registered nurse who is certified as a case manager and is an expert in post-acute care and insurance benefits. He or she will work closely with your physician and your family to ensure that all needed services are in place at home.

Once home, you will receive care tailored to your needs by experienced and trustworthy clinicians and caregivers who are also fully screened and insured.

Paying for Home Care

Many home care services are covered by Medicare or your health insurance plan. Staff will work with you and your insurer to help you get the services you need.

If you need care in the home that is not covered by medical insurance, our Private Care services provide day-to-day support to help you live independently. Services are usually paid for privately, though some long-term insurance plans cover private care.
